These roles will involve working with a wide range of customers and their carers to ensure they receive the right input at the right time and are directed to the right place.
You will receive referrals from the Hospital Assessment Team, Gateway to Care and the Wellbeing Hubs as part of the Self-Directed Support Pathway. Care Navigators utilise a strengths based approach to work with customers to enable them to remain independent in their own homes and local communities. Through appropriate sign posting, timely advice, sourcing and arranging support to meet their specific needs. Care Navigators will also be instrumental in supporting the roll out of the online financial assessment you will be expected to support this piece of work.
