We have a number of vacancies across our Mental Health and All Age Disability Services, and we are inviting applications from existing Approved Mental Health Practitioners (AMHPs), Experienced Social Workers and those who are willing to undertake the AMHP qualification.
We are welcoming applications from newly qualified social workers / ASYE’s, who will be supported to complete the Assessed and Supported Year in Employment progression.
Kirklees Mental Health and All Age Disability Services (including the Specialist Adult Pathway Team) provides strength-based and person-centred interventions for people with complex mental health and/or Learning Disability needs, with SAPT supporting young people through their Preparing for Adulthood journey as they transition into Adult Social Care.

What you will do:
As a Social Worker you will be responsible for a caseload within our integrated Community Mental Health Teams or within the All-Age Disabilities service, which will involve you undertaking holistic strength-based assessments and commissioning personalised support plans.
You will be expected to provide a least restrictive and a preventative approach to service users to avoid hospital admission to ensure people remain in their homes.
As an Approved Mental Health Professional, you will undertake Mental Health Act assessments with service users within the community.
You will provide an important and valued professional function working within a multi-disciplinary team with some of the most vulnerable people in society.
Your professional expertise and approach will ensure individual needs and aspirations are always supported, risk is managed proportionately, and safeguarding is of paramount importance.
You will be part of a dynamic, responsive and innovative team with varied tasks and no two days are the same.
The Specialist Adult Pathway Team (SAPT) is part of the All Age Disability Team.
The role of SAPT is to work with young people aged 14-25 years and their families with a focus on preparing them for adulthood.
This involves close liaison with the Children with Disabilities service and other Children’s Social Care teams.
What you will need:
You will require working knowledge of the Care Act, Mental Health Act, Human Rights Act, SEND legislation, Mental Capacity Act and associated Codes of Practice to fulfil your duties in this role.
You will need to have experience of working with the service user group and be able to use social work methods to promote positive change, independence, and wellbeing in accordance with assessed need, whilst promoting choice and control for adults and carers.
You must hold a Social Work England recognised qualification and have evidence that you are registered with Social Work England and be subject to a DBS check.
We will also consider applications from candidates who are in the final year of their social work degree.
Travel is an essential part of this job, and you will be required travel across Kirklees.
